Passport Information

Documents Needed to Process a Passport

  1. Passport Application, which can be obtained at travel.state.gov.
  2. Certified copy of birth certificate (must have a raised seal - hospital certificates are not acceptable). Birth certificates are mailed with your application but returned to you separately from your passport. Birth certificates can be obtained from your local health department. Birth certificate should be long form certificate with parental information. How to obtain an out of state birth certificate.
  3. One passport photo. This can be made at any location that handles passport photos. Example: The Package Store, Walgreens, etc.
  4. A check or money order payable to USDOS for Passport Book Fees.
    Passport Book Fees (Routine Processing)
     - Adults: $130.00
     - Minors: $100.00
    Passport Book Fees (Expedited Processing)
     - Adults: $190.00
     - Minors: $160.00                                                                                                                                                  - You may also pay an additional $21.36 for 1-2 day return delivery with expedited processing
  5. Passport Card Fees - cards cannot be expedited
     - Adult: $30.00
     - Minors: $15.00
    *NOTE: If you want 1-2 day delivery of your Passport book to your home/mailing address, you can pay for this service at our office.
  6. Acceptance Agent Fee: Separate check, Money Order, or cash payable to the Todd Greeson for $35.00
  7. Photocopy front and back of Driver's License or photo ID.

Please check the State Department website: travel.state.gov, for current processing times for passports. Currently processing times are 6-8 weeks for routine service, 2-3 weeks for expedited service.

The Circuit Clerk’s Office is located at the DeKalb County Courthouse.  Passport office hours: 8:00 a.m. to 4:00 p.m., Monday through Friday, excluding federal and state holidays. Closed for lunch 12-1 pm.

You may obtain a certified birth certificate from the Bureau of Vital Statistics from the state where you were born.  Follow this link to passport services if you do not know how to contact the state of your birth.  With a credit card, they will overnight your birth certificate to you.  Anyone born in Alabama may obtain a birth certificate at the DeKalb County Health Department:

Their office hours are 7:30 a.m. until 5 p.m.  If you want your birth certificate the same day, you must be there no later than 4 p.m.  The cost is $15 per certified copy; additional copies are $6 each.  Passport Services will not accept a hospital certificate or a notification of birth registration.

Several local photographers and businesses make passport photographs. (Walgreens, The Package Store) The Package Store, located at 601 Gault Ave. North Fort Payne, Alabama, is the preferred place.

If you wish to expedite, an additional fee of $60.00 will be required + $21.36 IF you want1-2 day return delivery back to address you provide.

Your passport and birth certificate will be mailed to the address provided on your application.

The application must be signed by the applicant before the passport agent in the Clerk’s Office, and the applicant must present a photo I.D. such as a driver license.  Each applicant, regardless of age, must appear.

Both parents (if both listed on birth certificate) must appear and sign the application for applicants under 16 year of age and present a photo ID and proof of relationship (such as a birth certificate) to the applicant.  If a parent cannot appear in person a notarized statement of consent must be presented.  The form is available in the Clerk’s Office or you can find it on travel.state.gov.

Passport books can be renewed if you submit your most recent passport that was issued on or after your 16th birthday, if it was issued within the last 15 years.  The Clerk’s Office has renewal forms available. The renewal fee is $130.00. Passport card renewals are $30.00 for adults. You may do this yourself.
